Winter Registration Form

Application is now available for those seeking to join any Leagues in the new year.  For further information about this, please click on Manual Winter Application Form. Copies are available at the Club. 

Please note the following:

1. Registration is not complete until a fully completed, legible and signed application is received by the Club with payment.  
2. Please consider E-Transfer when paying your membership fees. It's quick & easy, and saves the Club costly processing fees. CLICK HERE for instructions. Otherwise it's cheque by mail addressed to Welland Curling Club  PO Box 282, Welland ON. L3B 5P4, or payment at the Club by cheque, debit or cash (No credit cards).
3. On signing the application you have acknowledged and accepted the Welland Curling Club's  
   1. Release of Liability, Waiver of Claims & Indemnity Agreement(for applicants18 years of age and older),
   2. Informed Consent and Assumption of Risk Agreement (for applicants under 18 years of age. Parent/Guardian signed copy must accompany the Membership Application),
   3. Parents of applicants under the age of 18, and applicants age 18 to 25, acknowledge having reviewed Ontario's Rowan's Law Concussion Awareness Resourse.
